Lymphoma is a type of cancer that develops when white blood
cells grow abnormally. Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma
(CLL/SLL), Hodgkin Lymphoma (Hodgkin's disease), and Non-Hodgkin Lymphoma (NHL)
are the three primary forms of lymphomas (NHL). Hodgkin Lymphoma (Hodgkin's
disease) is less common and involves Reed-Sternberg cells, whereas lymphomas
that do not contain Reed-Sternberg cells are classified as Non-Hodgkin Lymphoma
(NHL) and are more common than Hodgkin Lymphoma.
After a period of remission, the phrase "relapsed"
refers to a disease that emerges or grows anew. When lymphoma does not respond
to treatment (meaning the cancer cells continue to proliferate) or when the
response to treatment is short-lived, the term "refractory" is used.
Patients with Relapsed
or Refractory Diffuse Large B-cell Lymphoma who have resisted or relapsed after initial chemotherapy can be
treated with high-dose chemotherapy followed by stem cell transplantation. The
majority of patients who get stem cell transplants will receive autologous
cells (patient receives his or her own stem cells, collected prior to the
procedure). A patient may require an allogeneic transplant on rare occasions
(patient receives stem cells from a donor).
A huge converted B cell with a nucleus diameter greater than
twice that of a normal lymphocyte is referred to as a diffuse giant B cell.
DLBCL is classed under the diagnostic heading of "mature B cell
neoplasms" in the 2008 WHO classification. DLBCL is a unique group with
numerous subgroups and entities based on morphology, immunophenotypic features,
and clinical presentation. The most prevalent type of DLBCL is DLBCL Not
Otherwise Specified (NOS). Cell-of-origin investigations using gene-expression
profiling revealed at least three different subtypes of DLBCL: activated B-cell
(ABC), germinal centre B-cell (GCB), and main mediastinal DLBCL. The presumptive
stage of cell genesis, gene expression, and responsiveness to
anthracycline-based treatment are all different. The response rate of GCB DLBCL
is higher than that of ABC DLBCL.
